Leeds Shared Lives (formerly Family Placement) offers carers of adults with a physical or learning disability or carers of older people the opportunity to have a break from their caring responsibility. We aim to help in a variety of ways

  • In crises and emergencies
  • By offering regular periods of care
  • Providing opportunities to acquire new interests or skills
  • By encouraging greater independence

What is home based care?
This is a free service provided in your own home by one of our approved carers. Our Shared Lives carer will come to your home and provide support to the cared for person so that you can have a break. This may be giving support with personal care like washing and dressing or providing supervision or guidance. Support can be offered:

  • During the day or evenings
  • Overnight

Who can use the service?

  • Carers in need of a break or urgent help
  • People with dementia
  • People with physical disabilities
  • People with learning disabilities
  • Frail, older people
  • People who are terminally ill

Who are the Shared Lives carers?
Our carers come from a wide variety of backgrounds and are all carefully recruited, checked and trained before they begin providing care to someone. There is no charge for this service.

How does the home based scheme work?
Referrals to the home based scheme are received from either a Social Worker, Care Manager or health care professional. Once an appropriate match is made the referring worker will introduce the Shared Lives carer to you.

This initial meeting is an opportunity for you and the person you care for to meet the Shared Lives carer and decide whether or not you would like to go ahead with placement. Only when all parties are happy to proceed will the placement go ahead. Any placements offered will depend upon your needs and circumstances and the person you care for.
